I use a 1/0 Wide Gap Gamakatsu hook and a 1/16 sinker.

I would wacky rig it with a 1/0 hook or less - Texas - maybe a 1/0 hook at most

for such a small worm I would go with something under a 3/0. You could probably get away with using a 3/0 but its pretty big for three inches, a 1/0 or 2/0 would be my preference. Sometimes when I'm using these in a pond or something I even use the bps size 1 hooks, which is the next step below a 1/0A:
I would use a 1/0 hook, or even better a 1/16 oz. to 1/8 oz. leadhead.A:
