I can see how it attaches to the worm but where does the eye of the hook go if you have the weight screwed into the end of the worm?

A: 
When you Texas rig your worm, just slide it 1/4 inch or so past the eye of the hook so the screw stops when it touches the worm rather than the eye. You'll need to bring the hook farther down into the worm before you turn it out.

Push the worm about 1/2 inch above the eye and screw the weight down into the worm.

This is not a Texas rig. The weight stays attached to the soft plastic of your choice. Just pull the eye of the hook further down into the soft plastic so the weight can screw into the soft plastic.

I've used them on all types of worms and I use the red ones on Money Craws or red worms. You could probably use lure paint to match them up to about anything. I also tip my plastics with JJ's Magic dye in chartreuse, methiolate or clear, and it is a killer combination.

Yes, The weight staying attached to the soft plastic gives the tail or claws more upward action which drives the bass crazy.
