You would push it into the head of a tube before you put your hook in.
These are not made for worms.

to put them in a worm just push em in the worm i like to put them up by the eye of the hook , as the fish tend to not hit that high up they tend to not break the rattler. i take a small tube of superglue and put them in tubes. you only need a tiny drop to hold the rattles in the tubes

the force of pressure at 500 ft will crush these as they are not meant to go more than 50 feet or so ... they are intended for fresh water, not the depths of the sea
